Vital Records

What are Vital Records?

Vital records are documents that certified major events in one’s life (i.e. birth, death, and marriage certificates etc.). All youth in out of home care should have a copy of their birth certificates.  This service is provided through the Maryland Department of Health.  

Get Your Vital Information for Your Personal Record

Before you leave foster care, make sure you have these essential documents for your records: checkbox Letter from the Director checkbox Birth Certificate checkbox Social Security Card checkbox Maryland State Identification Card (Required) or Driver’s License (if applicable) checkbox Medical Insurance Card checkbox Last Known Address of Parents/Relatives checkbox Placement History checkbox Child Welfare History checkbox Medical Records checkbox Education Records checkbox Credit Report Your caseworker will assist you with getting these documents, so contact your caseworker if you need any of these items or if they were lost or stolen.
